如何填写CB测试证书申请书

1.用英文（打字）填写CB测试证书申请书。
  2．中国电工产品认证委员会英文名称应填写在申请书首页右上部的四个小黑点之间。
  3。申请日期要填写清楚。
  4。如果申请人与制造厂是同一单位就分别在栏内填写相同的名称和地址。
  5．整机中使用的已获得CB测试证书的元件栏内，如有，应填元件名称及其cB证书号。
